  • The Consulate reserves the right to reject attestation of documents the contents of which are objectionable or contrary to the Rules.
  • The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India ) are required to be presented to authorities in USA, these should be apostilled in India. Important Update: With the opening of CGI Seattle, CGI San Francisco now serves the states of California, Arizona, Nevada, Utah, Hawaii, and Guam. Residents of Alaska, Idaho, Montana, Nebraska, North Dakota, Oregon, South Dakota, Washington, and Wyoming should submit their applications to CGI Seattle.

Applying In Person:

All miscellaneous/attestation services are now accessible on a walk-in basis, without the need for an appointment, from 9:00 AM to 12:00 PM on regular working days.

It is mandatory to apply for Indian passport of the Child alongwith registration of birth. The registration of birth must be done at the Indian Consulate within one year of Child's birth.

Parents of the child have to register birth online at indiancitizenshiponline.nic.in/ including uploading the picture. A print out of the registration done online needs to be self-attested by both parents and the same has to be attached with the miscellaneous service application seeking registration of birth at the Consulate.

Supporting Document: Carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application).  In case you are not able to present the original documents at the counter bring notarized copy of documents being attached to the application. Ensure that notarized copy is legible, otherwise Consulate may ask to see the original. However, the passport and the document (which is to be attested) should be presented in original.

Fee: A non-refundable fee of $ 25 (Twenty five) + $2 Indian Community Welfare Fee (ICWF) w.e.f. Oct. 01, 2012 per registration. If you want to receive the attested documents back through mail, send a trackable pre-paid return mailing envelope (USPS 'Express Mail' or UPS etc.) (No Fedex)

How to Apply:

  • Miscellaneous Services Form - Download the Miscellaneous Application form  and complete the application form.
  • Attach the following documents with the application form.
  • Notarized copy of Passport (First and Last Page)
  • Proof of US Visa Status of both parents:(Notarized copy of any one of the following)
    • Photocopy of the page containing visa on passport (H1B, H4 etc) and the copy of I-94
    • Clear Photocopy of Green Card
    • Employment Authorization Document (Work Permit)
    • I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being taken to regularize status) 
  • Proof of current US residence address for any parent: (Notarized copy of any one of the following)
    • U.S Driving license.
    • PG&E, Water or landline telephone bill displaying applicant's address
    • House Lease Agreement
    • State Identification Card
    • Note: Bank/credit card/mobile phone statements are not accepted as residence proof
  • Original Birth Certificate of the child alongwith a photocopy.
  • Notarized Declaration of both parents that they have not taken any other nationality for the child.
  • Self Sworn Affidavit (NOC) of both parents that they have no objection to the registration and obtaining passport for the child.
  1. In case the Consulate receives applications with incompletedocumentation, the same would be sent back unprocessed in the returnenvelope sent by the applicant.

Processing Time :

  • Applying inperson:
