Registration of Birth

Note:

• At least One of the Parents must be an Indian citizen.

• The Registration of Birth must be done at the Indian Consulate within One Year of the child's birth.

•  Once the birth registration is done, it is mandatory to apply for an Indian passport for the child.

Checklist of Documents to Bring to the Consulate

1.

Miscellaneous Application (Click Here)
Both the father and mother should fill, Print & Sign the form and submit it to the consulate along with the supporting documents

2.

Fill in Birth Registration form (Click Here )

Both the father and mother should sign the form.

3.

Fill in Self Sworn Affidavit (Click Here )

4.

Fill in Self Sworn Affidavit (NOC) (Click Here )

5.

One Notarized copy Birth Registration Certificate of the child issued by the local authorities

Original Birth Certificate

7.

Parents have to register the birth online at MHA’s Website (Click Here bullet)
A print out of the registration done online needs to be self-attested by both parents

Notarized Declaration of both parents that they have not taken any other nationality for the child.

9.

Originals + Copy of :

· Passport of the Parents

· Proof of US Visa status of the Parents

· Marriage Certificate of the Parents

· Proof of USA Address

Fees: $25 + $2 (Please issue 2 Separate Cashier’s Checks/Money Orders)

Note: Personal Checks/Cash/Credit Cards/Debit Cards are not accepted.
