Attestation of Death Document/Carrying of Ashes

  • The Consulate reserves the right to reject attestation of documents the contents of which are objectionable or contrary to the Rules. 
  • The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India ) are required to be presented to authorities in USA, these should be apostilled in India. Important Update: With the opening of CGI Seattle, CGI San Francisco now serves the states of California, Arizona, Nevada, Utah, Hawaii, and Guam. Residents of Alaska, Idaho, Montana, Nebraska, North Dakota, Oregon, South Dakota, Washington, and Wyoming should submit their applications to CGI Seattle.

Applying In Person:

Applicants can walk-in on any working day of the Consulate. Submission Time (9:30AM - 12:30PM)

Carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application).  In case you are not able to present the original documents at the counter bring notarized copy of documents being attached to the application. Ensure that notarized copy is legible, otherwise Consulate may ask to see the original. However, the passport and the document (which is to be attested) should be presented in original.

Delivery of Documents (Pick-up Hours) :  04.00 pm to 04.45 pm (Monday to Friday)

Applying by Mail:

Photocopy of all supporting documents should be notarized and attached to the application. Do not send any original documents except the PASSPORT and the document to be attested.

Fee:

For Issuance of NOC (Including Attestation of Death Documents):

For Indian passport holder: Gratis (No Fee) (for carrying mortal remains or ashes).
For US passport holder: Fee of $ 40.00 and US$ 2.00 towards ICWF (if carrying ashes)
Fee of $ 60.00 and US$ 2.00 towards ICWF (if carrying mortal remains)

For Attestation of Death Certificate: A fee of US$10.00 (Ten) and US$2.00 (Two) towards ICWF. For each additional copy a fee of US$10.00 (Ten) is applicable.

Fees should be paid through Money order or Cashier's cheque drawn in favor of the 'Consulate General of India, San Francisco'NO BLANK CHECK/MONEY ORDER WILL BE ACCEPTED BY THE CONSULATE. Cash, Personal cheques, Credit Cards, Debit Cards or other banking instruments are not accepted.

 

How to Apply:

  • Miscellaneous Services Form - Download the Miscellaneous Application form  and complete the application form. The next to kin or any family member[immediate relative] may sign the form and mention their relationship with the deceased person. A copy of his/her passport, visa status and proof of address along with a document that proves the relationship with the deceased person is required.
  • Current passport of the deceased (either Indian or US) in original and photocopy of the first five pages and last two pages of the Indian passport and/or photocopy of first two pages of US passport.
  • To carry Mortal Remains: All the death documents to be attested (Original Death Certificate, embalming certificate from the mortuary, burial/transit permit, all originals and a copy of each. No need to notarize.)
  • To carry Ashes: All the death documents to be attested (Original Death Certificate, Cremation Certificate; all originals and a copy of each. No need to notarize.)
  • Applicant may also ascertain with concerned Airlines about additional documentary requirements if any, and obtain these from the mortuary/funeral home.
  • If you want to receive the attested documents back through mail, send a trackable pre-paid return mailing envelope (USPS 'Express Mail' or UPS etc.) (No Fedex)
  • In case the Consulate receives applications with incomplete documentation, the same would be sent back unprocessed in the return envelope sent by the applicant.
